The Constellation Approach: taking action for hard-to-reach, isolated families proposes three working components, which encompass eleven success factors. To ensure successful outcomes, all three components must be taken into consideration when implementing measures and actions.

The first working component -Families’ needs at the heart of interventions- implies undertaking actions that reflect the values of the organization. This applies to the way the work is structured, as well as the relationships between the management, intervention workers and the families.

Know your territory and available resources

Having a clear idea of the socioeconomic conditions of the populations in the territory served, as well as of the available services, makes it possible to offer personalized guidance as well as intervention service continuity.
Maintaining a presence in the families’ living environments offers the opportunity to gradually get closer to and approach hard-to-reach, isolated families.

Diversify contact opportunities

Focusing on informal interactions and diverse meeting contexts provides opportunities to enter into contact with families of various profiles and needs.
Stepping out beyond the confines of the organization makes it possible to meet families who may have never before heard anything about the activities available to them.

Create a welcoming experience

A well-appointed space and the availability of staff to welcome families helps foster a warm and welcoming first contact experience and instills a sense of being in the right place.
Showing that you are open to the various requests and needs families may have will encourage them to come back.

Build relationships based on trust

The gradual development of a relationship based on mutual trust and respect, listening and empathy helps pave the way for the implementation of suitable and time-appropriate interventions.
When intervention workers are prepared to question their status as experts, they become more open-minded to the multiple realities experienced by hard-to-reach, isolated families.
